Symptom: The car brakes hard with nothing in the way. It usually happens with Autopilot or traffic-aware cruise on, sometimes with only the safety features active. Common triggers are overpasses, shadows, and large oncoming trucks on two-lane roads.

Likely causes

  1. Driver-assist software misreading the road. NHTSA opened an investigation (PE22-002) into 2021–2022 Model 3 and Model Y after complaints piled up. Our search results show it was closed in 2026 as reports dropped.
  2. One bad software release led to recall 21V846, which was fixed over the air.
  3. Camera view. Grime on the windshield in front of the cameras can make it worse.

Cost

Software fixes are free and arrive over the air. Camera service goes through Tesla.

DIY or shop?

  • DIY: keep the software updated, clean the windshield in front of the cameras, and file a bug report from inside the car.
  • Tesla Service: camera calibration or a hardware check.

Useful steps

  1. Note the date, time, location, and software version, and save the dashcam clip.
  2. Update the software and recalibrate the cameras if Tesla suggests it.
  3. Report it to NHTSA at nhtsa.gov/report-a-safety-problem.

Vehicle focus: 2020–2024 Tesla Model Y.

Safety / mechanic note

Sudden hard braking can cause a rear-end crash. Be ready to press the accelerator, and verify with Tesla Service if it keeps happening.
